Submit by Mail or Hand Delivery

To submit a paper Opt-Out Form, send your completed form with an original signature via First-Class Mail, overnight courier, or hand delivery to:

You may also submit your Opt-Out Form electronically by visiting the Opt-Out Portal at https://forms.stretto.com/EddieBauerLL/Opt-OutForm

Follow the on-screen instructions to complete and submit your form.

Important:
The Opt-Out Portal is the only accepted method for electronic submission.
Opt-Out Forms sent by email or fax will not be accepted.
If you submit your form online, do not also send a paper copy.
