W9 Submission Request

To: Allowed General Unsecured Claim Holders in the Bankruptcy Cases of Nashville Senior Care, LLC. and its affiliated debtors[1] (collectively, the “Debtors”)

 

To facilitate the distribution process in the event you are, or become eligible for a distribution, we request that you promptly complete an IRS Form W-9. In order to receive a distribution, you must provide the completed Tax Forms to the Trustee by no later than January 19, 2026.

Please note, the password to submit your W9 form is listed on the slip sheet included with your notice letter.

 

If you are a foreign party, one of the various iterations of the IRS Form W-8 must be completed. To do so, please reach out and request the applicable form at NashvilleInquiries@stretto.com.
