Upon completion of a proof of claim form (together with any supporting documentation you may have), please remit to the following address:

Creditors who have visited the Bankruptcy Court’s website will note that it provides for the filing of proofs of claim with the Clerk’s Office, either electronically or by mail. The Bankruptcy Court has approved the retention of Stretto as the claims and noticing agent for the Court in this bankruptcy case. Accordingly, proofs of claim filed with the Bankruptcy Court will be forwarded to Stretto and updated to the list of claims appearing on the “Search Claims” tab.
IMPORTANT NOTE REGARDING PERSONALLY IDENTIFIABLE INFORMATION IN YOUR PROOF OF CLAIM OR SUPPORTING DOCUMENTATION: Federal bankruptcy law requires that whenever a document (including a proof of claim) is filed with the bankruptcy court, the document must show (i) only the last four digits of an individual's social security, tax identification, or financial-account number, (ii) only the initials of a minor's name, and (iii) only the year of any person's date of birth. PLEASE MAKE SURE THAT ANY SUCH INFORMATION IN YOUR PROOF OF CLAIM AND SUPPORTING DOCUMENTATION IS PROPERLY DISCLOSED, INCLUDING BY MASKING OR DELETING ANY IMPERMISSIBLE NUMBERS OR NAMES. PLEASE REVIEW YOUR PROOF OF CLAIM AND SUPPORTING DOCUMENTS CAREFULLY PRIOR TO SUBMITTING THEM. PROOFS OF CLAIM ARE PUBLIC DOCUMENTS THAT WILL BE AVAILABLE FOR VIEWING ONLINE.
