On May 1, 2023 (the "Petition Date"), Legacy Cares, Inc. (the "Debtor") filed a voluntary petition for relief under Chapter 11 of the United States Bankruptcy Code. The Debtor's bankruptcy case is assigned Case No. 23-02832 before the Honorable Daniel P. Collins in the United States Bankruptcy Court for the District of Arizona (the "Bankruptcy Court").

On June 21, 2024, the Court entered the Order Confirming Amended Plan of Liquidation (the “Confirmation Order”) confirming the Amended Combined Disclosure Statement and Plan of Liquidation (the “Plan”).

The Plan became effective on August 1, 2024 (the “Effective Date”).
