Not Represented by an Attorney? READ THIS
If you are not represented by an attorney, it is very important that you send an e-mail with the following information to edmondstvl@gmail.com. We will be sending out instructions for attending the meeting and need this information to send you instructions:
1. Your name and case number.
2. Your meeting date and time.
3. Your e-mail address.
4. Your phone number.
Whether you are the Debtor filing Bankrutpcy or a Creditor owed money there is a great deal of relevent information on the Court Website at:
Creditors can file a claim electronically from this website. Please provide attachments to your claim to support it (when incurred, why the debt is owed, documents signed). The on-line claim filing will allow upload of pdf attachments with your claim.
If you are a former employee of a business debtor and you are filing a claim for unpaid wages, please attach a redacted (black out your social security number and bank information) copy of your last pay advise (pay stub/check stub) to validate your claim for wages owed. You can claim pre-petition hours and vacation time/PTO earned. Sick time is not compensable. For any commission you believe you are owed you will need to supply some additional documentations. There are no fines or late fees considered.

Please have your client(s) read the BANKRUPTCY INFORMATION PAMPHLET before their examination. I will ask if they have read it at the meeting.

PLEASE PROVIDE THE FOLLOWING DOCUMENTS AT LEAST 5 DAYS PRIOR TO THE MEETING OF CREDITORS
1. The most recently filed state and federal tax returns;
2. A copy of a valid, government issued picture identification card (i.e., state issued driver’s license or identification card, or a passport); THE MEETING WILL NOT GO FOWARD IF THIS IS NOT PROVIDED PRIOR TO THE MEETING
3. A copy of a social security card, Medicare card, W2 or 1099 wage statement bearing the debtor’s entire social security number, employer identification number or tax identification number;
4. If the Debtor pays spousal support or child support, please have them complete the Domestic Support Obligations (DSO) Form – by providing the name, address and telephone number of the support recipient.
Attorneys - You will be asked to please confirm that you have personally verified the Debtor's identity and the identification documents adn to confirm that the documents match the name of the Debtor and the identification number that appeears on the notice of meeting of creditors.
IMPORTANT: Please note that the 341(a) Meeting of Creditors are subject to being reset IF I am contacted by a creditor who wishes to examine the Debtor after the video examination or telephonic examination of the Debtor has already taken place.
