If you submit a claim electronically, please do not also mail the form.
INVESTORS SHOULD NOT SUBMIT A PROOF OF CLAIM FORM TO ESTABLISH THEIR INTERESTS IN LEGEND. Any Proof of Claim filed by an Investor on account of an Interest SHALL BE DISALLOWED on the grounds, among others and without limitation, that it is duplicative of an Investor Statement or an Investor Objection and the procedures related thereto.
Should you wish to receive a date-stamped copy of your proof of claim form, please include a photocopy of the form and a self-addressed, stamped envelope.
